I accept PPO plans with out of network benefits with Blue Cross Blue Shield, Aetna, Cigna & Anthem. Please use the contact page if you would like to utilize sliding scale or the insurance providers listed above. I do not accept policies managed by American Specialty Health (ASH) but am happy to create a superbill for you to give to your provider
